PDF will not be acceptable) & also Submit your application form along with Required Documents in Room No.114, Lower Ground Floor, Ghalib Block, NUML Islamabad by Hand or by Post.

1) Filled Registration Form
2) Paid Challan Form Rs. 6,000/- (Original)
3) CNIC/Passport (Copy)
4) 2 x Passport Size Photographs (Blue Background) to be pasted on Forms
